It is impossible for Bene-D-Duct to know when you have hired a new employee or when one has left, unless you keep us informed. Contractually it is your responsibility to do so.

Please have the eligible employee complete and mail us an employee form (click
Adobe Employee Form to download)or email us an electronic form (click Online form to fill out), indicating whether an addition or deletion is occurring, or if a family member is being added or deleted.

Bene-D-Duct will confirm the receipt of the form to the contact person for your business, and amend our files and records. This way you will know we have received the Employee Form and acted upon it, as well, you can check for any inconsistencies and notify us accordingly.
